5. Travel Information
Purpose of trip: Select B1/B2 (BUSINESS/TOURISM) for the combined visa, which is the most flexible option. Enter your intended arrival date and how long you plan to stay. You need a US contact — this can be a hotel address or the person you are visiting.

Photo Requirements
